Mental Health Skills-Building Services (MHSS).

A service provided to adolescents and adults (age 19 and older) to enable individuals with significant psychiatric functional limitations to achieve and maintain community stability and independence in the most appropriate and least restrictive environment. Our counselors collaborate and coordinate with other case-involved service providers and professionals, such as; attorneys, school personnel, social workers, and probation counselors. This process also identifies community resources and services that can be of benefit to both the individuals and their respective families.

Additional Program Information

(a) The individual shall have one of the following as a primary Axis I DSM diagnosis: 

  • (1) Schizophrenia or other psychotic disorder as set out in the DSM,
  • (2) Major Depressive Disorder – Recurrent;
  • (3) Bipolar I; or Bipolar II;
  • (4) Any other Axis I mental health disorder that a physician has documented  specific to the mental illness… (complete eligibility definition provided upon  request)

(b) The individual shall require individualized training in acquiring basic living skills  (complete eligibility definition provided upon request) 

(c) The individual shall have a prior history of any of in-patient care (i.e.  hospitalization) (complete eligibility definition provided upon request) 

(d) The individual shall have had a prescription for anti-psychotic, mood stabilizing, or  anti-depressant medications within the 12 months prior to the assessment date.  (complete eligibility definition provided upon request)

Community Stabilization.

FSI provides Community Stabilization services for non-hospitalized individuals by providing direct mental health care to individuals experiencing an acute psychiatric crisis that may jeopardize their current community living situation. FSI goals of the Community Stabilization program is to avert hospitalization or re-hospitalization, provide normative environments with a high assurance of safety and security for crisis intervention, stabilize individuals in psychiatric crisis, and mobilize the resources of the community support system and family members and others for on-going maintenance and rehabilitation. Our Community Stabilization program shall provide to individuals, as appropriate, psychiatric assessment including medication evaluation, treatment planning, symptom and behavior management, and individual and group counseling.

Additional Program Information

Individuals must meet the following admission criteria:  

  1. Documentation of a current primary diagnosis (consistent with a severe and persistent mental illness) that seriously impairs functioning in the community. Individuals with a sole diagnosis of substance addiction or developmental disability are not eligible for services.
  2. The individual has demonstrated a level of acuity indicating that they are at risk for crisis cycling or dangerous decompensation in functioning (i.e. performing practical daily living tasks, maintaining employment at a self-sustaining level or consistently carrying out homemaker roles or maintaining a safe living situation) and require additional support in the form of community stabilization services.

Must also meet the additional criteria

A) The individual is residing in a Therapeutic Group Home or ASAM 3.1: OR

B) The individual needs community stabilization as a transition due to either i. OR ii. AND also meets iii. below:

i. Referral from an appropriate source (i.e. licensed professional at a CSB, MCO, or licensed professional at a private provider)

ii. Individual is being discharged from one of the below services:
a) 23-Hour Crisis Stabilization
b) Acute Psychiatric Inpatient Services
c) ASAM levels 3.1-4.0
d) Hospital Emergency Department
e) Short-term detention or incarceration
f) Mobile Crisis Response
g) Partial Hospitalization Program (Mental Health or ARTS)
h) Psychiatric Residential Treatment Facility
i) Residential Crisis Stabilization Unit
j) Therapeutic Group Home

iii. Individual meeting either criteria i. or ii. above must also meet the following additional criteria:
1. The service recommended by the appropriate referring professional is not currently available AND
2. The recommended service has been identified and a transition from Community Stabilization will be within two weeks.

Intensive In-Home Services (Coming Soon).

The Intensive In-Home Services (IIHS) is a rehabilitative and support service for individuals with primary Axis I diagnosis consistent with a serious and persistent mental illness. IIHS provides direct rehabilitation and support to individuals in the community with the intention of supporting the individual to maintain stable community living, and prevent exacerbation of mental illness and admission to higher levels of care. Foundation Stones, Inc. serves to help each individual realize his/her goals and works with them to achieve independence while fostering dignity and self-respect. Our multidisciplinary team conducts comprehensive assessments, establishes viable individual service plans, teaches life and job skills, and links residents to appropriate community resources.

Additional Program Information

  • Individuals must meet all of the criteria: Documentation of a current primary diagnosis (consistent with a severe and persistent mental illness) that seriously impairs functioning in the community.   • The individual has demonstrated a level of acuity indicating that they are at risk for crisis cycling or dangerous decompensation in functioning (i.e. performing practical daily living tasks, maintaining employment at a self-sustaining level or consistently carrying out homemaker roles or maintaining a safe living situation) and require additional support in the form of community stabilization services.

Diagnosis = Meet ONE

- mental, behavioral or emotional illness attributed to recent significant functional impairments in major life activities. 

At-Risk = Meet TWO

  1. Have difficulty in establishing or maintaining normal interpersonal relationships to such a degree  that they are at risk of hospitalization or out-of-home placement because of conflicts with family or  community; and/or 
  2. Exhibit such inappropriate behavior that repeated interventions by the mental health, social  services, or judicial system are necessary and the result is risk of out of home placement; and/or 
  3. Exhibit difficulty in cognitive ability such that they are unable to recognize personal danger or  recognize significantly inappropriate social behavior and the result is being at risk for out of home  placement. 

The child must meet at least two of the following criteria on a continuing or intermittent  basis: 

  1. Have difficulty in establishing or maintaining normal interpersonal relationships to such a  degree that they are at risk of hospitalization or out-of-home placement because of  conflicts with family or community. 
  2. Exhibits such inappropriate behavior that repeated interventions by the mental health,  social services or judicial system are necessary. 
  3. Exhibits difficulty in cognitive ability such that the child is unable to recognize personal  danger or recognize significantly inappropriate social behavior.

Services shall be used when out-of-home placement, due to the clinical needs of the child, is a risk and  either: 

  1. Services that are far more intensive than outpatient clinic care are required to stabilize the child in  the family situation; or 
  2. The child’s residence, as the setting for services, is more likely to be successful than a clinic. 

With respect to both 1 and 2, the IIH assessment must describe how services in the child’s residence are more likely to be successful than an outpatient clinic.

Assessment Components

General Assessment.

The assessment process will vary by program or service offered. Program assessments are generally completed by an LMHP or Licensed Mental Health Professional (or LMHP-Eligible). However, some programs may allow for a QMHP to complete assessments. Even in this case, assessments will be completed according to the same format of the most stringent program, besides the employee status.

Preliminary Assessment.

Prior to admitting anyone to FSI’s community or home-based services programs, the LMHP shall complete a preliminary assessment that is detailed enough to determine that the individual qualifies for admission and to develop a preliminary individualized services plan. The preliminary assessment shall include the following:

  • A description of the abuse or neglect, emotional difficulties, or behavioral difficulties that led to the referral;
  • A description of any treatment or service needs that have been identified by the referral source;
  • A brief description of the client and family’s behavioral, functional, and social strengths, if known;
  • A description of any past treatment or service efforts and their outcomes, if known;
  • The names of presently involved service providers and the names of past service providers, if known;
  • Any medications currently prescribed for the individual and the individual's history of medical care, if known; and
  • Information concerning any applicable legal status of the individual and family, including guardianship, relevant criminal charges or convictions, probation or parole status, and any legal finding or pending proceedings concerning abuse or neglect;
  • If an individual meets admission criteria the LMHP will develop a preliminary service plan, within 24 hrs.

Referrals.

The Foundation Stones team is excited to work with the Lynchburg area community. We are accepting Medicaid-based referrals.
